Maccabi has developed an advanced, integrated system for telemedicine which is already fully functional in four areas: x-ray, ultrasound, EKG and Holter EKG (24-48 hour cardiac monitoring). The system is totally computerized using a computerized Radiology Information System (RIS), a PACS for storage and retrieval of radiological images, and digital imaging equipment for tele-radiology and tele-ultrasound, and computer linked EKG and Cardiac Monitors for the tele-EKG and tele-"holter" systems.
As with the laboratory system, the physician sends an electronic order for the specific test which is entered into the system and can be accessed at the clinic where the test will be performed. Once the test is performed, it is sent digitally to a central interpretation center where there are expert radiologists (in the case of x-rays and ultrasound) or cardiologists (in the case of EKG and holter), who provide expert interpretation of the test results.
The digital image in the case of radiology tests allows the radiologist in the interpretation center to manipulate the image, including looking at it from different angles, enlarging the image or increasing the contrast, thus enabling him to make a more accurate diagnosis.
In the case of gynecological ultrasound, the interpretation of the test is usually done by the gynecologist performing the test on the spot rather than a central interpretation center, but the results, as in all of the other radiology tests, are stored in the digital archive which enables comparisons over time.
The results (and the image in the case of radiology tests) are then transmitted (usually within 20 to 30 minutes)to the clinic that performed the test, the referring physician's computer as well as to the online internet site so that it can be seen by the patient on his own computer. This system enables the patient to perform the test at any clinic anywhere in the country including remote locations and to receive the results almost instantaneously.
Maccabi's Urgent Care Centers are also linked into the system and can relay and receive results after hours for patients requiring urgent care after the regular clinics have closed.
