Maccabi's population is aging and the burden of chronic disease is increasing. As a health care delivery organization, Maccabi is at the forefront of innovation in managing the care for its chronically ill and elderly patients, including disease and case management and a growing use of telemedicine. Maccabi's management information system acts as a vital support system that enables its caregivers, at all levels of the organization, to manage these populations effectively and efficiently. In addition, it generates a data base that supports groundbreaking research in these areas. Examples of unique projects in this area include:
- "Health Passport", a project for empowering the elderly, involved primary care physicians and advancing physicians' knowledge and understanding of the needs of the elderly.
- A special health promotion module has been introduced to the system to allow tracking of health promotion indicators and reminders to those who have not performed certain screening measures.
- A computer based tool is being developed, using our vast data resources, to identify frail, at-risk elderly by pinpointing key information, such as age, hospitalizations, etc. and automatically alerting family doctors about those identified at-risk and encouraging them to perform comprehensive geriatric assessment.
The Maccabi Research Institute is involved in promoting research in areas such as:
- Technology and systems for personalized, point of care diagnostics and monitoring.
- Chronic Care Management and Self-Management
- Systemic solutions for independent living, inclusion and active ageing
- Health systems and long term care of the elderly
- Health outcome measures and the ageing population
- Improving successful integration of geriatric hospitalization with community services to increase continuity of care.
